Outreach Worker job, Full time, 37 hours, Gravesend
Your new role
To provide advice, assistance and support to existing or entrenched rough sleepers in East Kent to enable them to secure accommodation suited to their immediate needs (Caseload subject to the contract for area). To work within policies and procedures (Health and Safety, Personnel, Housing Services, Support Services, Outreach procedures etc.) upholding the values and ethos of the organisation. To engage with rough sleepers in East Kent and to thereby reduce the need for longer term rough sleeping in East Kent by providing practical and sustainable solutions to the individual's needs, with accommodation in either the supported or private rented sectors and on going support provided by recognised funded or accredited agencies as the most desirous outcome.
The successful candidate will also need to be able to cover other community project roles according to operational need, to include helpline and Community Link Teams to give information advice and guidance and to promote recovery and community participation of individuals with mental health needs by enabling them to proactively apply the 'five ways to wellbeing' recommendations in the Live It Well strategy.
Main Responsibilities
1. To engage rough sleepers in designated areas, through street contact, including early and late street outreach, maintaining a regular street presence and initiating contact with those sleeping rough. Ensure a record of each street outreach is kept so an overall profile of rough sleeping in the area can be produced.
2. To use an assertive outreach approach to those who are reluctant to engage with support services. This may include working closely with and sharing information with statutory agencies.
3. To interview and assess applicants and to keep all parties (e.g. referrers, applicants etc.) informed of progress and the outcome.
4. To make referrals to relevant support in order that rough sleepers with mental health or substance misuse needs are actively engaged in treatment.
5. To ensure that people with the appropriate entitlement are referred to the appropriate Local Authority Housing Department for long term accommodation where this suits their needs and is compatible with their reasonable preferences.
6. To ensure that rough sleepers referred to accommodation are given full and comprehensive information about the accommodation, including details of service available, rules of residence, length of possible stay and rent requirements, in order to ensure a high rate of successful retention.
7. To establish positive working relationships with service users and to provide guidance and assistance to the service user in drawing up a support plan to meet their housing-related support needs and support them in reaching their aims. To review and update the support plan regularly.
8. To carry out regular visits to service user housed within the private rented sector and support them in maintaining their tenancy through advocacy, liaison, advice and referral to more specialised agencies as appropriate. This may sometimes involve an element of 'crisis management.' To keep up to date and accurate records of visits.
9. To ensure that service users housed within the private rented sector, and other housing, are enabled to maintain stable accommodation through referral to Floating Support schemes.
10. To ensure that rough sleepers are made aware of relevant provision in the area, e.g. day centres, food runs, advice services.
11. To regularly report, through supervision, on progress to the Project Manager and Operations Manager, to ensure a high level of support is given to service users,
12. To maintain accurate and up to date financial and administrative records and systems and assist in the compilation of monthly performance reports and other reports as required.
13. To attend and facilitate weekly drop - ins, administer relevant housing advice, refer/signpost to appropriate services and liaise with relevant local authority
14. To promote appropriate service user involvement in the delivery of the service and ensure that service user views are heard, responded to and reasons for decisions are given as soon as possible.
General
15. To undertake and participate constructively in induction, regular supervision, appraisal and relevant training, and contribute positively to good team relationships and continuous improvement of services.
16. To attend project and other meetings as directed by the (Outreach) Project Manager & Operations Manager. 17. To maintain confidentiality in line with charity policy in relation to service users, staff and business sensitive information
