Urgent Care Team Co-ordinator
Full Time 37 hours per week plus flexibility for on-call rota
£20,000 per annum DOE
Based at Beltinge, Herne Bay

Crossroads Care Kent is a Charity offering a range of supportive, practical services to Carers
and the people they care for. We now have an exciting opportunity to grow your career in Social Care by joining our Urgent Care office team.

Our Urgent Care team is responsible for the operational delivery of quality urgent care to carers and their families across the County and supporting hospital discharge services.

Duties will include liaising with families, supporting a team of urgent carer support workers, co-ordinating and rostering calls, undertaking assessments, preparing care plans and participating in an on-call rota. Working with health and social care professionals, including hospital discharge teams, you will be looking to ensure best possible outcomes for people.

Our role would suit an organised, flexible, patient and caring team player with a can-do attitude, committed to making a difference to the lives of carers and the people they care for. Experience of staff rostering and co-ordination, working to deadlines and undertaking care assessments is essential along with a working knowledge of care practice. Great communication, robust admin and keyboard skills, with the ability to conduct risk assessments and agree safe service delivery are also key for this role. Admin or people management experience,
an NVQ Level 3 or higher in Social Care would be an advantage.

A full driving licence and use of own car are essential for travel across Kent areas as required.
