You’ll then make it happen by supporting them to navigate and access health, social care and community services. It’s a varied role, and you’ll spend time visiting people in their own homes or in community settings, speaking to individuals and families. You’ll also organise and deliver group activities for adult Carers, bringing them together to experience peer support and take a break from their caring roles. Are you ready to make a difference? This is an exciting opportunity to join our Navigation Centre team who provide essential support to our services working with people in communities. The team is the first point of call for all communications, so you must be confident communicating with clients, families and social care professionals by phone and email. Based in a fast-paced office, the tea deal with a constant flow of referrals and enquiries and you will be trained to use our bespoke database. Ability to follow processes, excellent data entry skills, speed and accuracy are key. Due to the nature of the role, you will need to swap between phone calls, enquiries and tasks while remaining focused. You will prioritise and allocate tasks to support workers in the field, as well as providing information and signposting people to other services and organisations. This is a short-term post and offers a great opportunity to learn about Social Prescribing whilst supporting individuals impacted by long-term health conditions. Long COVID affects people physically, mentally and emoti nally. People experience it differently, and as a condition, it is still relatively new to patients and professionals. In this role, you will signpost and refer patients to services so they can better self-manage their condition and its impact. You’ll coordinate activities for patients through six weekly sessions, progressing them through their journey and initiating interventions.
