We’re looking for a bright, hardworking individual to join our established service in Dartford, Gravesham & Swanley. It’s a great opportunity to support adults who need extra help for all sorts of reasons to consider what wi l best improve their health and wellbeing. You’ll then make it happen by supporting them to navigate and access health, social care and community services. It’s a varied role, and you’ll spend time visiting people in their own homes or in community settings, speaking to individuals and families. You’ll also organise and deliver group activities for adult Carers, bringing them together to experience peer support and take a break from their caring roles. Navigators bring their own experience to the role. You might have worked in a formal setting, completing assessments and support plans, or directly providing social care or health services.
