We are looking for an organised, self-motivated and confident individual to join our Social Prescribing team working with adults and children, specialising in mental health, and helping patients to access support that will help them lead more independent, resilient and healthier lives. Working alongside GP practices, Social Prescribing Link Coordinators help chil and adult mental health patients and families connect with health and community services which will make a real difference to their emotional and physical wellbeing. You will deliver personalised support to patients, independently managing new referrals from health professional and a caseload.
